Echoing Stranger here, this poem doesn't work well because it ruminates on the very baseline of ideas about death. It touches on being scared of death, the unknown of what would happen after, and the afterlife, but nothing is said here that is unique in its perspective. What does death mean to you? What are your experiences with death?
Additionally, (and this is not a hard rule for all poetry) a lot of poems like this will have some sort of twist at the end to get the reader to think about the poem in a different way than how they had been for the majority of reading. This does not do this very well either, as the last stanza is a pretty natural progression of the thought-process presented in the rest of the poem.
